For TK/K entry only, visit your doctor and submit the completed Report of Health Examination for School Entry form.
If your child is entering 7th grade, please provide updated TDAP booster vaccination records to the school office.
If your child requires medicine to be administered at school:
- Download a copy of the Administration of Medicine at School form for each medicine required
- Bring to school on the first day in a clear plastic bag with your student’s medicine enclosed and their name clearly labeled on the bag.
If your child has any food allergies, please complete a FARE Emergency Plan for your student and return a copy to the school office prior to the first day of school.
